Nursing Home Transitions
The Nursing Home Transition Program supports individuals that are in a nursing facility, are at risk for long term placement, and wish to return to their community. Georgia’s Department of Human Services/Division of Aging Services contracts with the Area Agency on Aging and Centers for Independent Living to administer the Nursing Home Transition program. This program is specifically for individuals who are 55 years of age or older. The program does not require Medicaid and there are no housing or asset restrictions. To make a referral for the Nursing Home Transition program, please contact your local Area Agency on Aging or the MDSQ-Options Counselor for your local county.
ELIGIBILITY REQUIREMENTS
- Minimum of 20 consecutive day stay in a nursing facility
- Proof of citizenship (current photo ID or birth certificate)
- Income must be below $6,175 per month
- Must meet Level of Impairment requirements (assessment will be completed)
