Elderly & Disabled Waiver Program (EDWP)
Services are designed to meet the individual needs of people who are aging or who have disabilities that are Medicaid Eligible to allow people to remain in their home and community with the supports they need.
Transition Services: Options Counseling & Nursing Home Transition
Options Counseling, Nursing Home Transition, and Transition Services allows people residing in in-patient facilities who are elderly or have physical disabilities to move back into the community and receive support services to live in their own homes and communities.
